Russian Federation blames rocket failure on mistake during assembly

A day earlier, Sergei Krikalyov, a senior Roscosmos official, told reporters a malfunction of a sensor that signals the jettisoning one of the rocket's four side boosters caused the booster to collide with the second stage of the rocket.

Russia's space agency, Roscosmos, has released footage from a camera on board the Soyuz rocket that failed last month, forcing a dramatic emergency landing of the two astronauts on board.

That target date falls less than eight weeks after the October 11 launch anomaly, which forced the Soyuz spacecraft carrying NASA astronaut Nick Hague and cosmonaut Alexey Ovchinin to make an emergency landing on the steppes of Kazakhstan.

The two men landed safely on a steppe in Kazakhstan; the incident is the first of its kind for Russia's manned programme in over three decades.

Russian Federation suspended all launches after the accident on October 11, unprecedented for Russia's post-Soviet manned launches, that saw the rocket fail minutes after blast-off.

"The cause of the abnormal separation was the failure to open the lid of the exhaust nozzle of the oxidizer tank of the "D" block due to the deformation of the stem of the contact separation sensor committed during assembly of the "package" at the Baikonur Cosmodrome", he said. More recently, Russia's space program has been dogged by a string of failed satellite launches involving unmanned vehicles.

The sensor was damaged during the rocket's assembly at the Baikonur spaceport in Kazakhstan, the probe commission head said.

"We have a number of Russian Soyuz rocket launches in the next month and a half and in December, we're fully anticipating putting our crew on a Russian Soyuz rocket to launch to the International Space Station again", NASA administrator Jim Bridenstine said last week.

They warned that two other Soyuz rockets could be defective, and said additional checks have been introduced.

An uncrewed Soyuz ISS mission will precede the December 3 liftoff, Russian space officials added today: A Soyuz will launch a robotic Russian Progress cargo ship toward the orbiting lab on November 16.

But the failure is worrisome, since Soyuz is the only human-rated spacecraft used to get people to and from the space station.

Russian space officials say they hope to resume sending crews to the International Space Station on December 3 after an October launch failed because of a technical malfunction. Russian Federation stands to lose that monopoly with the arrival of SpaceX's Dragon and Boeing's Starliner crew capsules.
